Driving Directions
Located within the True Life Medicine building, approximately two blocks from Highway 24. Turn at McDonald's onto Baldwin Street, heading north. The building appears on the right after passing Skyline Drive. Enter the driveway, turn left, and proceed uphill. The office entrance and parking are located on the upper level at the rear of the building.
Payment Information
Full payment is required at time of service. We accept:

- Credit/Debit Cards (3% processing fee)
Insurance
While we are not in-network with insurance companies, we will submit out-of-network claims for chiropractic benefits on your behalf. Please contact your insurance provider to verify your out-of-network chiropractic coverage.
New Patient Forms
Download and print the patient intake form, complete and sign it, and bring it to your visit. If you can't complete it beforehand, please arrive 10-15 minutes early.
